Job Summary

CareAlliance LLC is seeking a skilled LPN to join our team as a Community Health Specialist. In this role, you will provide high-quality, comprehensive medical and dental care to patients in need, regardless of their ability to pay.

Key Responsibilities
  • Collaborate with healthcare teams to provide supportive medical care to clients in various settings, including mobile clinics and community health ambulatory care settings.
  • Provide nursing care, health promotion, patient and family support, health maintenance, disease management, coordination of care, and maintaining continuity of care.
  • Work at outreach and community locations throughout Cleveland, providing care on a mobile clinic to patients with various social determinants to healthcare.
  • Professionally provide treatment to patients who may be homeless, in correctional facilities, or other controlled environments.
  • Triage patients, schedule follow-up care, and referrals using EPIC.
  • Perform basic health screenings and report pertinent findings to management or providers.
  • Implement appropriate components of the plan of care, under the direction of the healthcare team.
  • Assure all needed POCT are completed and documented in EMR prior to provider visit.
  • Work collaboratively with other Care Alliance health professionals to facilitate a team approach to patient management.
Requirements
  • HS diploma or GED and completion of a state-approved LPN nursing program.
  • Current license to practice as an LPN in the State of Ohio.
  • Minimum of 2 years' experience as a LPN, with experience in a community outreach environment preferred.
  • Comfortable working with patients in various settings, including people who are in outreach clinics, shelters, encampments, and homeless.
  • Good problem-solving, computer, verbal, and written communication skills.
  • Must be able to efficiently use EPIC.
  • Current BLS Certification.
