Overview of Responsibilities

The Hospice Nurse Case Manager operates under the general supervision of the Senior Clinical Manager-Homecare and collaborates closely with the attending physician to deliver skilled nursing assessments, planning, and care aimed at enhancing the comfort and well-being of patients and their families. This role involves identifying the needs of patients and families while providing supportive care in alignment with the physician's directives, the established care plan, and the policies and procedures of Teleios Collaborative Network, including necessary documentation and timely completion of all required paperwork. The nurse may also serve as a visiting nurse or case manager.

Key Responsibilities:

  • Advocate for the hospice philosophy.
  • Take primary responsibility for a caseload of patients and families, encompassing the assessment, planning, implementation, and evaluation phases of the nursing process.
  • Initiate and maintain communication with attending physicians, hospice team members, and external agencies to ensure optimal care and resource utilization for patients and families.
  • Regularly update the Senior Clinical Manager-Homecare regarding caseload status.
  • Communicate consistently with the attending physician about the status of patients and families.
  • Gather information on physical, psychological, social, and spiritual factors that may affect patient and family health, integrating this data into the care plan.
  • Keep accurate and current patient records, ensuring that problems, plans, actions, and goals are clearly documented and updated as necessary.
  • Seek input from team members regarding patient care plans to enhance knowledge and support.
  • Assume responsibility for coordinating the physical care of patients by educating primary caregivers, volunteers, and employed caregivers, or by providing direct care as appropriate.
  • Participate in regular meetings with hospice clinical staff to discuss challenges or unique issues from caseloads, share professional support, and exchange feedback to foster professional growth.
  • Attend patient conferences to facilitate discussions on caseload issues for comprehensive staff evaluation and consultation.
  • Supervise and revise the health aide care plan bi-weekly or more frequently as needed.
  • Notify the Senior Clinical Manager-Homecare of any unusual or potentially concerning patient or family issues.
  • Respond to the needs of patients and families of other hospice nurses during on-call hours or as directed by the Senior Clinical Manager-Homecare.
  • Provide necessary support during the time of death and throughout the bereavement period.
  • Take responsibility for personal nursing practice, ongoing education, and professional development.
  • Encourage the involvement of patients and families in the care plan, with a focus on pain and symptom management, as well as emotional and spiritual support.

Qualifications:

  • Registered Nurse with a current license in the State of Maryland.
  • A minimum of 2 years of diverse nursing experience, with hospice experience preferred.
  • At least 1 year of experience in an acute care setting. New registered nurses may be considered for employment upon agreement to participate in and complete the Preceptor Program.
  • Proficient in nursing practice, capable of managing family emotional stress, and respectful of individual lifestyles.
  • Depending on experience and training, the hospice nurse may be assigned to perform assessments for various specialties including pediatrics, obstetrics, and psychiatry.

Work Schedule:

  • Full-time, Monday to Friday, 8:00 AM to 4:30 PM.
  • Participate in on-call rotations, providing 24/7 coverage for patients and families requiring symptom management and emotional support.

FLSA Status:

This position is classified as exempt under the Fair Labor Standards Act for full-time employees.
