Primary Care Nurse Practitioner

Position Overview:

Utilizing a solution-focused methodology, this role delivers comprehensive primary care aligned with professional training while overseeing clinical performance to ensure that medical services and operations comply with all relevant regulatory and licensing standards. Additionally, this position acts as a mentor and trainer for clinical support personnel, providing direction in best practices and addressing medical service challenges based on urgency in accordance with the policies of Covered Bridge Healthcare of St. Joseph County.

Key Responsibilities:


• Fulfill all specific obligations and execute tasks as delineated in individual agreements and modifications.


• Conduct thorough physical examinations and maintain accurate medical records.


• Perform comprehensive physical assessments, identifying normal ranges and managing the resolution of abnormalities while initiating appropriate treatments.


• Order necessary diagnostic tests, including imaging, laboratory analyses, screening procedures, and other specialized evaluations, ensuring timely communication of results to patients and coordinating follow-up care as required.


• Gather patient histories and evaluate risk factors, such as hypertension, diabetes, substance use, and other medical, physiological, and psychological conditions.


• Involve patients and their families in care planning, offering educational support and advocacy on health-related matters.


• Engage in and promote community health education and outreach initiatives.


• Collaborate with physicians, nurses, physician assistants, and other healthcare providers within the practice and the broader community.


• Consult with physicians regarding complex cases and the development of patient care plans.


• Participate in ongoing record evaluations and quality improvement initiatives.


• Contribute to the health center's quality assurance program, adhering to established protocols and guidelines.


• Follow universal precautions and comply with OSHA and workplace safety regulations.


• Complete all necessary documentation, including progress notes, problem lists, medication records, allergy information, dictations, insurance forms, and quality assurance records, in a timely manner.


• Assist in the formulation and execution of the health center's healthcare strategy.


• Maintain patient confidentiality and obtain necessary releases of information as required, adhering to HIPAA regulations.

Required Skills and Qualifications:


• Awareness of ethical considerations in the staff-patient dynamic.


• Commitment to confidentiality and adherence to HIPAA policies and procedures.


• Appreciation for and understanding of cultural competence.


• Strong dedication to serving underserved and vulnerable populations within the community.


• Commitment to the mission of Covered Bridge Healthcare.


• Familiarity with preventative medicine principles and practices.


• Knowledge of state and federal regulations governing medical practice and community health centers.


• Basic understanding of health clinic operations and processes, with a preference for knowledge of Federally Qualified Healthcare Center (FQHC) operations.

Education and Experience:


• Graduation from an accredited nursing program.


• Completion of an accredited Nurse Practitioner Program.


• Current Nurse Practitioner Certification in Family Medicine.


• Valid Medical Licensure in the State of Michigan.


• Current DEA registration and Controlled Substances Licensure (preferred).


• Basic Life Support (BLS) certification.

Work Environment:

This position requires the ability to sit and stand for extended periods and involves working in an office setting with some physical activity, including bending, walking, stretching, and lifting. Adherence to strict timelines and deadlines is essential.
