Physician - Chronic Disease Management

We are seeking a highly skilled Physician to join our team at Yukon-Kuskokwim Health Corporation. The successful candidate will be responsible for effectively managing chronic disease, building relationships with patients, and providing excellent care in a collaborative team environment.
Key Responsibilities- Manage chronic disease through clinical management, patient education, and activation.
- Work collaboratively with physicians and other healthcare professionals to provide excellent care.
- Develop and maintain positive relationships with patients, co-workers, and others.
- See a large spectrum of patients in clinic and hospital settings.
- Medical Degree (MD or DO) and completion of an ACGME accredited residency program.
- Current licensure as a physician in an appropriate state and current certification in CPR.
- Successful completion of credentialing at Yukon-Kuskokwim Health Corporation.
- High level of customer service skills.
- Ability to work collaboratively in a team environment.
