Nurse Case Manager

Mass General Brigham is seeking a skilled Registered Nurse (RN) to join our Case Management team in Peabody, Massachusetts.
Job Summary:We are looking for a dedicated and compassionate RN to provide exceptional care to our patients. As a Case Manager, you will be responsible for assessing patients' needs, developing and implementing transition plans, and facilitating communication between healthcare providers.
Responsibilities:- Conduct initial assessments and develop transition plans for patients
- Participate in multidisciplinary rounds to ensure comprehensive care
- Perform utilization reviews to optimize patient outcomes
- Advocate for patients and their families
- Delegate tasks to Case Management assistants as needed
- Current Nursing License in the State of MA
- Three or more years of clinical experience in a healthcare delivery system
- Completion of an accredited Nursing program
- Culturally competent and able to work in a diverse environment
- Case Management, Home Care Discharge Planning experience
