Nurse Practitioner

Mass General Brigham Healthcare at Home is seeking a skilled Registered Nurse to provide high-quality clinical services to patients in various settings in the community. The ideal candidate will have a strong background in acute care and experience working with patients with complex needs.
The successful candidate will be responsible for delivering patient care within the organization's philosophy, policy, and standards of community health practice. This will involve thorough and timely electronic documentation, interdisciplinary communications, and case management responsibilities in accordance with MGB Healthcare at Home policies and standards of practice.
The ideal candidate will possess excellent interpersonal and customer service skills, with the ability to work effectively with a diverse population. Strong communication skills, both written and verbal, are essential for this role. The ability to work independently, prioritize tasks, and adapt to changing situations is also crucial.
Key Responsibilities:
- Provide high-quality clinical services to patients in various settings in the community
- Deliver patient care within the organization's philosophy, policy, and standards of community health practice
- Perform thorough and timely electronic documentation and interdisciplinary communications
- Manage cases in accordance with MGB Healthcare at Home policies and standards of practice
- Work effectively with a diverse population and possess excellent interpersonal and customer service skills
Requirements:
- Graduate of an approved School of Nursing
- Current Massachusetts license as Registered Nurse
- Minimum of one year previous nursing experience in acute care setting
- Prior home care experience preferred
- Ability to work with various computer software and knowledge of hand-held patient care computer devices preferred
