Home Health Nurse Specialist
About the JobLHC Group, a leading provider of post-acute care services, is seeking a skilled Home Health Nurse Specialist to join our team. As a Home Health Nurse Specialist, you will play a critical role in delivering high-quality, patient-centered care to individuals in their homes.
Responsibilities:- Conduct comprehensive assessments of patients' physical and emotional needs
- Develop and implement personalized care plans to address patients' unique needs
- Provide direct nursing care to patients, including administering medications, wound care, and other treatments as prescribed by physicians
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless coordination of care
- RN licensure in the state of Louisiana
- Minimum one year of experience in home health or hospice care
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Estimated annual salary: $80,000 - $110,000 based on experience
