We are seeking a skilled Nurse Practitioner to join our hospice team as a PRN provider. As a member of our interdisciplinary team, you will play a critical role in delivering high-quality patient care and supporting our mission to provide compassionate and personalized care to those in need.
Responsibilities- Perform Face-to-Face (F2F) encounters and gather clinical information to support hospice diagnosis and prognosis.
- Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility.
- Conduct physical assessments and provide clinical recommendations to the IDG.
- Notify the hospice agency when unable to complete a scheduled F2F visit.
- Master's or higher degree in Nursing.
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
- Current state license as a registered nurse.
- Current state license or certificate as an advanced practice nurse.
- Current national certification as a Nurse Practitioner (ANCC or AANP).
- Collaborative agreement in place.
