As a Vocational Nurse II, you will engage in complex nursing tasks that require a high level of expertise.

Your responsibilities will include caring for individuals at the State Hospital (SH) who present with intricate mental health, behavioral, and medical conditions.

You will perform both routine and advanced nursing duties to promote the well-being of individuals during your assigned shifts.

You will collect specimens for laboratory analysis and conduct health assessments. Additionally, you will guide psychiatric nursing assistants and other staff in delivering care that meets the unique needs of each individual. You will execute technical nursing procedures in accordance with facility and state regulations, ensuring compliance with all agency and hospital policies. You will also provide mentorship and training to new LVNs and direct care staff.

In collaboration with Registered Nurses (RNs), you will assist in implementing protocols and programs aimed at enhancing the health and safety of both staff and individuals.

You will work under the general supervision of an RN, utilizing your initiative and independent judgment effectively.

You may also be assigned additional responsibilities as needed.

These may include participating in disaster response efforts or supporting Continuity of Operations (COOP) initiatives, which could require adjustments to your shift patterns or work location.


Key Responsibilities:

1. Maintain regular attendance and be prepared to work specific or rotating shifts, including extended hours or overtime as required by agency policies.

2. Administer medications and treatments via various methods, including oral, rectal, and injection.

Collaborate with other healthcare professionals, such as RNs and Physicians, to assess and care for patients.

Conduct rounds as necessary and be flexible to work in different areas of the hospital as needed. Demonstrate proficiency in executing complex nursing tasks in compliance with HHSC, hospital, and state regulations.

Train and orient new LVN staff to ensure they are well-equipped to meet the needs of the individuals.

3. Carry out advanced nursing procedures as per facility and state guidelines, including wound care and respiratory treatments. Collect specimens for lab testing and perform necessary screening tests as requested.
4. Monitor individuals' dietary intake, weight, and vital signs. Assess health status following medication administration and treatments. Instruct psychiatric nursing assistants on monitoring protocols post-treatment.

Accurately document and report relevant treatments and observations in the individual's medical record, adhering to hospital protocols.

Recognize and report symptoms of illness to the RN or Primary Care Provider.
6. Promote interdisciplinary collaboration and communication.

Share important information with team members, including the RN, to ensure informed decision-making within the treatment team.

7. Act as a resource and mentor for new LVNs, assisting the RN Supervisor in training and instruction.
8. Support RNs in implementing health and safety protocols, assisting in infection control measures, and contributing to quality assurance initiatives.

Gather data for RNs and Primary Care Providers as needed.

9. Provide education to individuals in line with their recovery plans, explaining medications and treatments as necessary.
10. Ensure the availability of necessary supplies for procedures and treatments, maintaining inventory and ordering as required.
11. Respond to emergencies to ensure the safety of individuals, adhering to hospital safety and infection control protocols.
12.

Fulfill additional duties as assigned, including participation in disaster response or COOP activities, which may involve shift changes or relocation.


Required Knowledge, Skills, and Abilities:
1.

Familiarity with nursing care standards, procedures, and techniques, as well as principles of patient care.

Knowledge of emergency protocols and equipment, including CPR, with the ability to assess and respond to emergencies effectively.

2. Strong problem-solving skills and the ability to reason through complex situations.
3. Proficiency in computer operations and relevant software applications.
4. Expertise in patient care and treatment methodologies.
5.

Ability to provide care to individuals with diverse medical, mental health, and behavioral challenges.

6. Competence in record-keeping and familiarity with electronic health record systems upon training.
7. Effective communication skills, both verbal and written, with the ability to instruct others.
8. Ability to interact appropriately with individuals with intellectual and developmental disabilities, their families, and staff members.
9. Current CPR certification is required, along with the physical capability to perform CPR and related tasks.
