About the Role

We are seeking a highly skilled and compassionate Senior Social Care Specialist to join our dialysis team at Health Systems Management, Inc. This is an exciting opportunity for a dedicated professional to work in a dynamic healthcare environment, providing high-quality patient care and support.

Job Responsibilities

The successful candidate will be responsible for:

  • Providing direct and indirect interventions to pre-dialysis and chronic dialysis patients;
  • Collaborating with the multidisciplinary healthcare team to develop and implement comprehensive care plans;
  • Communicating effectively with patients and their support systems to establish and maintain care plans;
  • Conducting thorough psychosocial assessments to identify patient needs and develop strategies for support;
  • Utilizing community resources to meet patient and family needs;
Requirements and Qualifications

To be considered for this role, candidates must possess:

  • A Master's degree in social work (MSW) from an accredited institution;
  • Licensure as a social worker in the state of Georgia;
  • Excellent communication and interpersonal skills;
  • The ability to work independently and prioritize tasks effectively;
  • A willingness to work a flexible schedule and adapt to changing circumstances;
