Job Summary:The Family Nurse Practitioner will be responsible for assessing, diagnosing, and treating patients with a wide range of medical conditions. You will work collaboratively with our healthcare team to develop and implement individualized treatment plans, ensuring that each patient receives the best possible care.
Responsibilities:- Provide comprehensive primary care to patients of all ages, including health promotion, disease prevention, and management of acute and chronic conditions.
- Conduct thorough physical examinations, take medical histories, and order diagnostic tests as needed.
- Develop and implement treatment plans, including prescribing medications and ordering treatments.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Maintain accurate and up-to-date patient records, adhering to all relevant laws and regulations.
- Master's degree in nursing from an accredited program.
- Current RN licensure in the state of Indiana or eligibility for licensure.
- Nurse Practitioner certification in Family Medicine or a related field.
- At least two years of experience as a Family Nurse Practitioner or in a related field.
- Valid driver's license and reliable transportation.
