Position Overview:

Bridges Outreach, Inc. recognizes the vital role of Outreach Specialists in the mission to eliminate homelessness. We are seeking compassionate individuals with a solution-oriented mindset and meticulous attention to detail and data integrity.

Under the guidance of the Program Manager, the Outreach Specialist will navigate the community to identify and deliver essential services to individuals facing unsheltered homelessness. The ideal candidate will possess prior experience working with chronically homeless populations.

The Outreach Specialist will engage with individuals experiencing mental health challenges and substance use disorders, fostering trusting relationships with those who have faced homelessness for extended periods and have limited access to resources. This role is pivotal in connecting street homeless individuals to necessary services and assisting in the development of personalized goal plans aimed at improving overall health, securing housing, and achieving independence.

This position calls for a proactive individual with exceptional interpersonal, communication, and organizational skills. Most importantly, Bridges Outreach seeks a dedicated person with a genuine desire to serve. Additionally, Outreach Specialists will play a key role in coordinating our weekly Outreach Runs, which includes training, onboarding, and supporting volunteers involved in outreach efforts.

Available Shifts:

• Full-time, part-time, and per diem opportunities are available.

• Shift 1: 12:00 am to 8:00 am (Days: Sunday, Monday, Friday, Saturday)

• Shift 2: 4:00 pm to 12:00 am (Days: Sunday through Saturday, TBD)

• Shift 3: 8:00 am to 4:00 pm (Days: Saturday and Sunday only)

Key Responsibilities:

  • Engage daily with individuals experiencing homelessness, building supportive and trusting relationships while focusing on targeted goals.
  • Utilize non-coercive, trauma-informed practices to establish trust and position oneself as a partner in their journey out of homelessness.
  • Encourage clients to access services for which they qualify to achieve their service plan goals.
  • Provide immediate transportation to clients for urgent services, including shelter or warming center transport.
  • Deliver mobile case management services, including:
    • Orienting clients to available services and their requirements.
    • Conducting initial intakes and assessments to identify clients' emergency and immediate needs.
    • Maintaining contact with referral sources.
    • Acting as a liaison between clients and service-rendering agencies.
    • Participating in field visits and transporting clients to medical, housing, mental health, or substance abuse treatment appointments.
    • Engaging clients in a supportive treatment partnership while maintaining professional boundaries.
    • Participating in collaborative case meetings with Project Connect staff.
    • Ensuring accurate data entry in a customized database.

Qualifications:

  • Proficiency in Google Suite and Microsoft Office (Word, Excel, PowerPoint); experience with HMIS is a plus.
  • Outstanding written and verbal communication skills.
  • Strong time management and organizational abilities; adept at managing multiple projects and prioritizing effectively.
  • Willingness to work with diverse identities and lived experiences.
  • Ability to lift 35 pounds.
  • Other duties as assigned.

Education:

Associate degree preferred or 2+ years of experience in a human services setting (including internships).

Experience:

  • Prior experience working with underserved populations is beneficial (e.g., mental health, drug abuse counseling, life coaching, social work).
  • Preferred: One year as a street outreach worker or homeless services case manager or related experience.
  • Experience in high client volume environments.
  • Sensitivity to economic and sociological barriers.
  • Crisis de-escalation certification is a plus.
  • BSW is a plus.

Requirements:

  • Valid driver's license with no points.
  • Ability to be insured under the company driver's policy.
  • 90% travel required (company vehicle provided).
  • Bilingual (English/Spanish) is a plus.

Job Type: Full-time/Part-time/Per-diem

Compensation: Starting at $24.00 per hour.

Schedule:

• Must be available to work an 8-hour shift, Sunday to Saturday.

• Availability to cover shifts as needed.

Relocation:

• Must be able to reliably commute or plan to relocate before starting work.
