Key Responsibilities

- Conduct initial assessments for new patients.


- Develop, coordinate, and update comprehensive care plans while ensuring continuity of care through collaboration with relevant staff.

- Regularly communicate patient progress to the clinical manager, physician, and care team.
- Maintain accurate and timely individualized patient care records.
- Document changes in pain and symptom management as necessary.
- Educate families regarding patient conditions, needs, and care plans.
- Refer patients to other care disciplines as needed, or provide rationale for not doing so.
- Fulfill on-call responsibilities as assigned.
- Perform additional duties as required.

Qualifications

- Current Speech Pathologist license specific to the state of practice.
- Minimum of one year of relevant experience required.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
