The Physical Therapist is responsible for delivering and overseeing physical therapy services to residents, ensuring comprehensive assessment, treatment, program development, and documentation. This role operates under the directives of a physician.
Key Responsibilities include, but are not limited to:
- Delivering physical therapy services to residents and initiating referrals as necessary in accordance with departmental protocols.
- Conducting regular screenings of residents to assess the need for therapeutic intervention.
- Evaluating residents to gather essential data for effective treatment planning and execution, including specialized assessments as required.
- Communicating evaluation results to residents, their families, and other healthcare team members.
- Coordinating individual resident schedules in collaboration with the healthcare team.
- Formulating treatment plans that identify problems, establish short and long-term goals, and outline methods to achieve these objectives.
- Collaborating with residents, families, and team members to coordinate treatment plans.
- Implementing and executing treatment as specified in the treatment plan.
- Monitoring residents' responses to interventions and adjusting treatment as necessary to meet established goals in accordance with physician directives.
- Creating suitable home or community programs to support residents in maintaining and enhancing their functional abilities.
- Documenting assessments, treatments, follow-ups, and service terminations accurately.
- Adapting treatment approaches to meet the unique needs of each resident.
- Providing recommendations and training on adaptive devices and equipment, including orthotics and prosthetics.
- Participating in discharge planning processes.
- Supervising and guiding Physical Therapist Assistants in executing treatment plans.
- Overseeing students and volunteers during their practical training.
- Managing Physical Therapy Aides and Rehabilitation Aides effectively.
- Reviewing the quality and appropriateness of services delivered and individual therapy programs for efficacy and efficiency.
- Engaging in educational activities for residents, families, and staff.
- Collaborating with team members to coordinate the overall rehabilitation strategy for residents.
- Adhering to departmental procedures for documenting physical therapy services.
- Attending required in-service and facility meetings.
- Providing educational sessions for care team members and the broader community.
- Taking responsibility for ongoing professional development and continuing education.
- Recording billable treatment times in accordance with departmental procedures and submitting statistical reports as required.
- Delivering treatment in line with established standards of practice and departmental guidelines.
- Maintaining a safe and organized work environment while adhering to company safety protocols.
- Complying with all infection control measures and OSHA standards relevant to healthcare professionals.
- Demonstrating sound judgment in evaluating, planning, implementing, and following up on resident therapy programs.
- Fostering positive relationships with coworkers, residents, families, and facility staff.
- Maintaining punctuality and adhering to work schedules while ensuring acceptable patient ratios and operational efficiency.
- Contributing to quality assurance initiatives as required.
- Upholding confidentiality standards as outlined in HIPAA policies.
- Projecting a professional and positive image at all times.

Minimum Qualifications
- Bachelor's degree in Physical Therapy from an accredited institution.
- Successful completion of the National Certification Examination for Registered Physical Therapists.
- Current state licensure or eligibility for licensure.
- Consistent attendance and reliability.
Physical Requirements and Work Environment
This position requires the ability to exert varying levels of force, with a classification of medium according to the Department of Labor. The employee must be capable of lifting and moving objects and residents as needed. Regular activities include standing, walking, sitting, kneeling, and using hands for manipulation of tools and equipment. The work is primarily conducted indoors, in resident rooms or clinical settings. This role is classified as Category 1 for exposure determination under OSHA regulations, necessitating the use of protective equipment.
