Program and Role Details:

Our Transitional Foster Care Program for Unaccompanied Children (UC), overseen by the Office of Refugee Resettlement (ORR), focuses on the safety and well-being of unaccompanied minors (ages 0-17) who arrive in the U.S. without an adult. Unlike traditional care settings, we place these children in foster homes while we diligently seek and assess potential sponsors. During their stay, we provide comprehensive medical care, case management, engaging life skills classes, and essential mental health services.

As a Practical Nurse Specialist, you will deliver the medical care required for the unaccompanied children we serve. We seek individuals who are passionate about assisting this vulnerable population and can thrive in a dynamic medical office environment. Your role is crucial to the health and well-being of these children as they navigate their new lives in the U.S.

Key Responsibilities:

  • Conduct initial medical assessments
    • Measure vital signs and perform point-of-care testing (e.g., urine pregnancy tests, strep tests, flu tests, COVID tests, glucose tests)
    • Review immunization records, prepare and administer necessary vaccinations
    • Update the City Immunization Registry
    • Administer medications, track medication administration records provided by foster parents, and assist with documentation for prescriptions
  • Serve as an English-Spanish translator for medical providers
  • Collaborate with the Medical Provider, Case Manager, Clinician, Medical Assistant, and Skills Trainer to coordinate services for each youth
  • Administer and interpret TB tests for youth
  • Ensure compliance with all medical reporting requirements, including government UCPortal and internal electronic medical records

Work Schedule:

  • Regular hours Monday to Friday with shifts available from 8:00 AM to 4:00 PM and 10:00 AM to 6:00 PM
  • Remote on-call duties may be required, including weekends and holidays
  • Flexibility is essential to meet program needs
  • Must hold a New York State Practical Nurse License
  • Experience working with youth is preferred
  • Strong communication and organizational skills are necessary
  • Bi-lingual in English and Haitian Creole is a plus
  • Ability to work effectively within a team environment
  • Demonstrated capability to perform duties with minimal supervision and exercise sound judgment

Physical Requirements:

  • Proficient in word processing, including email correspondence, document preparation, and familiarity with electronic medical record systems
  • Ability to complete paperwork, including copying, scanning, and faxing documents
  • Must be able to lift up to 50 pounds
