Position Overview:
The Southeastern Michigan Health Association is a prominent, community-focused organization dedicated to enhancing health outcomes within the local population. We are currently seeking an Early Intervention Services Coordinator to engage with individuals who have recently received an HIV diagnosis or those who have fallen out of care. This role is integral to our team of certified community health workers, responsible for facilitating access to essential services and resources for our clients.

Key Responsibilities:

  • Conduct program orientations for clients in diverse environments, including healthcare facilities and community outreach events.
  • Assess client needs through a strengths-based approach and collaboratively develop personalized care plans.
  • Maintain comprehensive client records, ensuring accuracy in demographics, health status, and progress documentation.
  • Assist clients with medical insurance enrollment and appointment scheduling, providing support as necessary to eliminate barriers to care.
  • Build and sustain a robust referral network within the healthcare and social services sectors, identifying available resources and addressing service gaps.
  • Facilitate client transitions to follow-up services, including medical case management.
  • Collect and report data for program evaluation and monitoring purposes.
  • Input and manage client data within our information systems.
  • Engage in multidisciplinary team meetings and case conferences to ensure coordinated care.

Qualifications:

  • Demonstrated interpersonal skills with a collaborative approach to problem-solving.
  • Strong organizational abilities and adeptness in managing multiple priorities.
  • Excellent verbal and written communication skills.
  • Proficiency in Microsoft Office and familiarity with social media platforms for client engagement.

Education Requirements:

  • High School Diploma or equivalent is required.
  • Community Health Worker certification is preferred; training will be provided.
  • Bilingual proficiency in Spanish is advantageous.

Preferred Experience:

  • Knowledge of HIV/AIDS and related health issues.
  • A minimum of three years of experience working with individuals living with HIV.
  • Possession of a valid driver's license and reliable transportation.
  • Availability to work evenings and weekends as needed.

Compensation:
$38,000 - $43,000 annually, based on experience and qualifications.
