Job SummaryWe are seeking a highly skilled Case Manager/Clinical Behavioral Rehabilitation Services (CBRS) Specialist to join our team. As a key member of our team, you will be responsible for providing comprehensive mental health services to clients in assisted living homes, ensuring timely and effective care.
Responsibilities- Conduct thorough assessments, treatment plan reviews, and progress notes to ensure accurate and effective care.
- Organize and maintain accurate documentation, adhering to payor and company standards.
- Collaborate with other treatment providers to ensure comprehensive care.
- Comply with HIPAA privacy and practices, maintaining confidentiality and professionalism.
- Assess emergency situations, notify supervisor, and/or appropriate personnel of actual and potential issues.
- Maintain a non-judgmental approach, effective listening, eye contact, professionalism, and positive interactions with clients.
- Provide consistency and support to promote personal progress and accountability.
- Bachelor's degree required, with a preference for Social Work.
- CPR certification preferred.
- Ability to pass an enhanced IDHW background check.
- Valid driver's license with reliable transportation and proof of car insurance.
- Ability to work in a community-based setting.
- Experience utilizing electronic health records preferred.
- Zero no-shows and minimal non-billable time, ensuring maximum billable hours.
- Stipend for non-billable hours, gas, and activities with clients.
