Job SummaryWe are seeking a skilled Community Based Rehabilitation Specialist to join our team. As a key member of our staff, you will provide skill building and service delivery to clients in accordance with their individualized treatment plans.
Key Responsibilities:- Provide skill building and service delivery in accordance with the objectives specified in the individualized treatment plan.
- Assist clients in gaining skills necessary to undertake school, employment, independence, and other issues related to mental health.
- Develop and maintain appropriate documentation for service delivery, treatment reviews, and staffing requirements.
- Work with diverse populations and maintain professional boundaries.
- Conduct assessments, crisis intervention, individual and family services.
- Maintain HIPAA and ethical standards.
- Bachelor's degree from a national accredited university or college.
- Ability to work with diverse populations.
- Responsible for timely completion of mandated paperwork and maintaining productivity standards.
- Flexibility and ability to manage ever-changing priorities.
- Must pass a criminal background check conducted by the DHW.
