**Job Summary**
Whitecap Search is seeking a skilled Nurse Practitioner to join our client's team in a nationwide primary care organization focused on the geriatric population. This is a full-time, permanent position with a comprehensive benefits package.
Key Responsibilities:
- Work collaboratively with a care team, including a scribe, nurse, and social worker, to provide high-quality patient care.
- Conduct 6-7 home visits per day, traveling independently and reimbursing costs.
- Provide primary care to patients in their homes or transition care services immediately following a hospital visit.
- Adhere to a value-based care model within a large managed care organization.
- Utilize a modern electronic medical record system and processes.
Requirements:
- Board Certification as a Nurse Practitioner.
- PA State License.
- Training as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (ANP).
- Minimum 1 year of experience in home care, nursing home, or primary care settings.
