Job SummaryWe are seeking an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients in the Bucks County, PA region.
You will work under the supervision of a physician to care for a diverse set of patients with chronic diseases.
Key Responsibilities- Provide high-quality care to primary and palliative patients in and around Greater Philadelphia (Montgomery and Bucks, PA)
- Assess patient health by performing physical examinations, obtaining medical histories, and creating personalized care plans
- Effectively communicate with office staff and clinical operations across various internal platforms
- Provide patient-centered care to a panel of patients
- Attend quarterly medical staff meetings
- Develop diagnoses and treatment plans
- Foster trust and consult with patients, families, and caregivers during visits
- Order diagnostic testing as medically necessary
- Prescribe medications, supplies, and interventions as needed
We are looking for a Board Certified Nurse Practitioner with a valid NP certification in the state of Pennsylvania.
You should have strong clinical skills, a positive demeanor, and excellent people skills.
A minimum of 1-2 years of primary care experience with geriatric patients is required, with hospice experience highly preferred.
