Job Summary

We are seeking an experienced and compassionate Nurse Practitioner to join our team in the Greater Philadelphia region. As a primary care specialist, you will provide high-quality care to homebound and palliative care patients, applying medical techniques and principles under the supervision of a physician. You will assess patients' health, create personalized care plans, and develop strong relationships with patients, families, and caregivers.

Key Responsibilities
  • Provide exceptional primary and palliative care to patients in and around Greater Philadelphia
  • Conduct thorough assessments, create accurate diagnoses, and develop effective treatment plans
  • Communicate effectively with office staff and clinical operations across various internal platforms
  • Develop strong relationships with patients, families, and caregivers
  • Order diagnostic testing and prescribe medications as medically necessary
Requirements
  • Board Certified Nurse Practitioner with a valid NP certification in Pennsylvania
  • Strong clinical skills and ability to accurately diagnose and treat patients
  • Positive demeanor and excellent people skills
  • Ability to work independently and confidently problem-solve
  • 1-2 years of primary care experience with geriatric patients (hospice experience a plus)
