Key Responsibilities- Preoperative Navigation: Contact patients prior to surgery to review their medical history, verify clearance letters, and ensure they have the necessary preoperative clearances.
- Postoperative Support: Call patients 1 day post-op to assess their progress, address concerns, and facilitate appointments to avoid emergency room visits.
- Documentation and Reporting: Enter detailed documentation for ER visits, complications, and readmissions in our navigation platform.
- Communication and Coordination: Respond to voicemails and Athena messages from patients, hospitals, skilled nursing facilities, physical therapists, home care, and physician clinical staff.
- Discharge Planning: Generate and send weekly discharge disposition reports to facilities, detailing patient plans and specific information for conveyance.
- Bachelor's of Science in Nursing (BSN) required.
- Minimum 5 years of experience in an orthopedic practice.
- Clinical and back-office experience preferred.
- Ability to follow up with patients and respond to inquiries promptly.
