About the Role

We are seeking a compassionate and dedicated Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will play a vital role in providing high-quality care and services to our residents and their families.

Responsibilities
  • Build meaningful relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Develop and implement Individualized Service Plans (ISPs) to ensure the unique needs and preferences of each resident are met.
  • Participate in the development of care plans and monthly updates to ensure the highest level of care is provided.
  • Collaborate with the community team to ensure compliance with OSHA requirements and promote a safe and healthy environment for residents and staff.
  • Provide exceptional customer service and respond to the dining needs of residents and guests.
Qualifications
  • Dedication to serving seniors with excellent customer service skills.
  • Positive attitude, flexibility, and ability to work in a team environment.
  • High School diploma/GED accepted and may be required per state regulations.
  • In states where applicable, must maintain certifications.
  • Must be at least 18 years of age.
  • Previous experience working with seniors preferred.
