Job Overview

SouthernCare, a leader in home care services, is seeking a compassionate Hospice RN / RN Case Manager to join our team. About the Role
  • As a Hospice RN / RN Case Manager, you will serve as the primary liaison between physicians, patients, and families, fostering open communication and collaboration.
  • You will deliver direct patient care services, empowering patients and their caregivers with education and resources to manage complex health conditions.
  • Your expertise will be instrumental in evaluating patient needs, developing comprehensive plans of care, and maintaining accurate clinical records.
About You

To succeed in this role, you must possess:

  • A genuine commitment to compassionate care and empathy for patients and their families.
  • Effective communication and interpersonal skills, with the ability to build strong relationships.
  • Strong critical thinking and problem-solving abilities, with a focus on patient-centered care.
  • RN licensure in the state of employment (or eligibility to obtain) and at least one year of experience in hospice, home health, or similar settings.
  • Preferred qualifications include experience in geriatric, oncology, or ICU settings.
