Advanced Practice Clinician

High Country Community Health Inc. is seeking a skilled Advanced Practice Provider to join our team. As a key member of our healthcare team, you will provide primary care services to patients, including assessing, diagnosing, prescribing, treating, and educating patients. You will work closely with physicians and other clinicians in a team approach to patient care, and may specialize in a specific type of clinical care, such as pediatrics or geriatrics.
Responsibilities
- Assess patient status by obtaining health history through patient/family interviews and chart reviews, and performing appropriate physical examinations.
- Order and perform laboratory diagnostic and other screening tests as needed, and seek consultation with physicians and other clinicians for evaluation of illness.
- Develop and implement treatment plans, including prescribing and dispensing medications and/or injections in compliance with medical practice guidelines and state laws.
- Provide patient education on health promotion and illness prevention, and recommend community resources to meet patient needs.
- Communicate case management information to other professionals and community agencies.
- Prepare documentation for medical records, including updating patient charts with examination and test results, diagnosis, medications, and treatment.
- Participate in peer reviews, chart reviews, staff education, and other continuing education and quality assurance activities.
Requirements
- MSN/Master's in PA Studies or equivalent.
- Successful completion of advanced practitioner training and certification as a practitioner (general or specialized) from an approved nurse practitioner or physician assistant program.
- Minimum three years of professional nursing/PA experience in a clinic setting.
- NC License to practice medicine/DEA and current CPR certification.
Work Environment
This position requires standing and walking for extensive periods of time, and occasional lifting and carrying of items weighing up to 10 pounds. You will work in a combination of exam rooms and medical offices, with frequent exposure to communicable diseases, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment.
