Position Summary:
St. Patrick Center, a dedicated ministry of Catholic Charities, is seeking a Community Support Specialist to be a vital part of the Assertive Community Treatment (ACT) program.
The Community Support Specialist will play a crucial role within the ACT team, delivering high-quality support services to individuals engaged with the program.
This role involves coordinating and overseeing the evaluation and treatment planning for each individual, ensuring follow-up services are provided, and acting as an advocate for client rights and entitlements.
Working Hours:
The standard working hours for this position are Monday to Friday, from 8:00 AM to 4:30 PM. The compensation for this role ranges from $30,000 to $40,000.
