Job Description:

The Homeless Services Coordinator position at Evergreen Treatment Services offers a challenging and rewarding opportunity to work directly with individuals experiencing homelessness and behavioral health conditions. As a key member of our Outreach and Engagement team, you will engage with clients in various settings, promoting harm reduction practices and providing education on safer drug use, overdose prevention, and accessing harm reduction resources.

Your Key Responsibilities:

  • BUILD TRUSTING RELATIONSHIPS WITH CLIENTS WHO MAY BE HESITANT TO ACCESS TRADITIONAL SERVICES.
  • CONDUCT BRIEF ASSESSMENTS TO UNDERSTAND CLIENTS' SUBSTANCE USE PATTERNS, MENTAL HEALTH STATUS, AND SOCIAL DETERMINANTS OF HEALTH.
  • DEVELOP INDIVIDUALIZED HARM REDUCTION PLANS IN COLLABORATION WITH CLIENTS, FOCUSING ON REDUCING RISKS ASSOCIATED WITH DRUG USE AND UNMET MENTAL HEALTH NEEDS.
  • COLLABORATE WITH EXTERNAL AGENCIES AND SERVICE PROVIDERS TO ENSURE COMPREHENSIVE CARE AND SUPPORT FOR CLIENTS.
