Primary Care Coordinator

Job Summary
We are seeking a primary care coordinator to join our team at Atria Physician Practice New York PC. As a registered nurse, you will work closely with our physicians to provide high-quality, patient-centered care to our members in the comfort of their own homes.
Your responsibilities will include:
- Collaborating with Atria physicians to develop and implement individualized care plans for our members.
- Conducting routine check-ups, administering medications, and conducting tests as needed.
- Maintaining accurate and up-to-date medical records for our members.
- Providing education and support to our members on various health-related topics.
Salary Range: $120,000 - $140,000 per year
Requirements:
- NYS Registered Nursing License.
- Minimum 2 years of experience as a practicing RN.
- BLS certification.
