Key Responsibilities
- Collaborate with patients, families, medical staff, and community agencies to determine the most appropriate setting of care and assess post-hospital care needs.
- Provide interventions to ensure effective and efficient clinical resource management, including adjustment to hospitalization, crisis intervention, and psychosocial assessment.
- Work with patients of all ages, from neonates to geriatrics, and their families to develop personalized care plans.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
- Graduate of an accredited Registered Nursing School (RN) with a current GARN license.
- One (1) year of experience in case management, utilization review, or social services.
- Excellent oral and written communication skills.
- Ability to type accurately at an approximate rate of 40-45 wpm.
Preferred Qualifications
- BSN degree.
- Three (3) years of experience in case management, social services, or a related field.
- Certification as a Certified Case Manager.
