We are seeking a skilled Registered Nurse to join our team as a Home Health Case Manager. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and coordinating services to meet the needs of individuals and families in their homes and communities.
Responsibilities- Conduct comprehensive assessments of patients and families to determine care needs
- Develop and implement individualized plans of care in collaboration with certifying medical providers
- Provide professional nursing care, including administering medications and treatments as prescribed
- Coordinate patient care for assigned caseload, acting as a Case Manager when assigned
- Collaborate with patients, families, and caregivers to address needs and meet related goals
- Graduate of an accredited school of nursing
- One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home health preferred
- Currently licensed as a Registered Nurse in the State(s) of planned practice
- Possesses and maintains CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
