Job Summary

We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at Signature Healthcare at Home. As a Hospice Registered Nurse Case Manager, you will play a vital role in providing high-quality patient care and support to individuals and their families in their homes and communities.

Responsibilities:

  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide professional nursing care, including administering medications, treatments, and other interventions as prescribed by physicians.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between healthcare settings.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
  • Supervise and delegate tasks to hospice aides as needed to ensure efficient and effective care delivery.

Requirements:

  • Current licensure as a Registered Nurse in the state of Oregon.
  • Graduate of a National League for Nursing accredited school of nursing.
  • Minimum of two years' experience in hospice or home care nursing, with a focus on community health education and experience.
  • Excellent observation, problem-solving, verbal, and written communication skills.
  • Ability to work independently and as part of a team to achieve patient-centered care outcomes.
