Job Title: Clinical Manager Home Health RN

Job Summary:

Signature Healthcare at Home is seeking a highly skilled and experienced Clinical Manager Home Health RN to oversee the clinical operations of our Home Health team. The successful candidate will have a strong background in nursing leadership, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Responsibilities:

  • Supervise and mentor a team of clinical staff to ensure high-quality patient care and adherence to regulatory standards.
  • Develop and implement clinical policies and procedures to ensure compliance with federal, state, and local regulations.
  • Collaborate with the leadership team to develop and implement business strategies to drive growth and improve patient outcomes.
  • Conduct regular audits and quality assurance activities to ensure compliance with regulatory standards.
  • Provide education and training to clinical staff on new policies, procedures, and technologies.
  • Develop and maintain relationships with community partners and healthcare providers to ensure seamless care transitions.
  • Participate in the development and implementation of the organization's strategic plan.
  • Perform other duties as assigned by the leadership team.

Requirements:

  • Current state license as an RN.
  • Two years of experience in a Home Health agency within the last five years.
  • Supervisory or management experience of professional clinical staff.
  • Experience with HomeCare HomeBase (HCHB) Software EMR.
  • Experience with Home Health Start of Care and Oasis Review, prefer Oasis Certification.
  • Working knowledge of state and federal home health regulations.
  • Excellent verbal, written, and computer literacy skills in Outlook, Word, and Excel.
