About the Role

Key Responsibilities
  • Assess and monitor client adjustment to community living, ensuring a smooth transition and optimal outcomes.
  • Monitor client participation and progress in organized treatment programs, ensuring timely and thorough communication with assessors and other professionals.
  • Develop and revise individualized treatment plans, incorporating the DLA-20 and timely communication with assessors regarding potential goals and objectives.
  • Provide individual assistance to clients in accessing needed mental health services, including accompanying clients to appointments and addressing medical or other health needs.
  • Assess clients' level of risks, strengths, and weaknesses, and interview clients, review records, and confer with other professionals to evaluate clients' preparedness for independent living.
  • Encourage medication compliance and provide education regarding medications and monitoring of side effects.
  • Provide individual assistance to clients in accessing a variety of public services, including financial and medical assistance and housing, including assistance on an emergency basis and directly helping to meet needs for food, shelter, and clothing.
  • Assist clients in accessing and utilizing community agencies and resources to provide ongoing social, educational, vocational, and recreational supports and activities.
  • Intercede on behalf of individual clients within the community-at-large to assist the client in achieving and maintaining their community adjustment.
  • Maintain contact with clients who are hospitalized and participate in and facilitate discharge planning.
  • Train, coach, and support clients in daily living skills, including housekeeping, cooking, personal grooming, accessing transportation, keeping a budget, paying bills, and maintaining an independent residence.
  • Assist in creating personal support systems that include work with family members, legal guardians, or significant others regarding the needs and abilities of a client.
  • Encourage and promote recovery efforts, client independence/self-care, and responsibility.
  • Provide support to families in the area, including treatment planning, dissemination of information, linking to services, and parent guidance.
  • Provide holistic, person-centered care.
  • Develop and support wellness and recovery goals in collaboration with the individual served.
  • Provide services to clients determined to be appropriate based on GAF score and level of care.
  • Perform job duties with an efficient use of time based on client needs and meet efficiency requirements based on budget parameters.
  • Notify the Program Director of any changes in client condition.
