RN Case Manager

Job Summary:
We are seeking a highly skilled and experienced Bilingual RN Case Manager to join our team at the University of Colorado Anschutz Medical Campus. The successful candidate will be responsible for providing patient care coordination, including helping patients connect with specialty care, providing clinical care for adults, and assisting families in accessing available community services.
Key Responsibilities:
- Provide patient care coordination, including helping patients connect with specialty care.
- Provide clinical care for adults, including assessing social and medical needs, and working with other providers to provide comprehensive and integrated care.
- Assist families in accessing available community services, including identifying beneficial programs and services for patients.
- Triaging patient phone calls throughout the day, providing guidance and support as needed.
- Document patient care using the electronic medical record system.
- Participate in community outreach events, promoting and building the integrated health team.
Requirements:
- Graduate with a bachelor's degree in nursing from an accredited school of nursing.
- Current RN licensure in good standing (any state).
- Ability to obtain Colorado RN license by date of hire.
- Ability to obtain current BLS/CPR certification by date of hire.
- Valid nation provider identifier (NPI) number by date of hire.
- Ability to remain credentialed and ensure credentialing materials stay current.
- Ability to work in a normal patient care environment with some exposure to biological hazards and infectious diseases.
- Ability to pass a sex offender background check.
Preferred Qualifications:
- Experience providing care coordination in a Federally Qualified Health Center, a public health setting, and/or to low-income families.
- Experience in program development and implementation.
- Case Management certification and/or 2 years or more experience with case management.
- Experience managing personnel.
Competencies:
- A considerable degree of independent judgment and work priorities is necessary to monitor and respond to changes in a client's condition.
- Interpersonal skills to work effectively with patients, their families, and other members of the interdisciplinary health care team.
- Skilled in the use of the electronic medical health record and other clinic software.
- Basic knowledge of Colorado state immunization requirements.
- HIPAA and OSHA compliance.
- Intermediate level computer competency.
- Exemplary organizational skills and ability to prioritize organizational needs.
- Ability to work independently and function collaboratively as a team member.
- Highly responsible, reliable, and courteous.
- Unconditional ability to maintain patient confidentiality.
- Leadership skills.
