Job Summary:This is a full-time, 36-hour per week position working 12-hour shifts. The successful candidate will have a Master's degree in Nursing and at least one year of experience in case management. Act 31 training completion is required within 45 days of hire.
Responsibilities:- Provide individualized care to patients with complex medical needs
- Develop and implement comprehensive care plans to promote optimal health outcomes
- Collaborate with interdisciplinary teams to coordinate patient care
- Conduct thorough assessments and evaluations to identify patient needs
- Develop and implement education programs to promote patient self-management
- Master's degree in Nursing
- At least one year of experience in case management
- Act 31 training completion within 45 days of hire
