Job Description

Bayview Hunters Point Foundation is seeking a highly skilled Senior Case Manager to join our team. As a Senior Case Manager, you will be responsible for overseeing and coordinating comprehensive case management services for individuals and families experiencing homelessness.

Key Responsibilities:

  • Manage a caseload of complex cases, providing direct support and intervention.
  • Conduct thorough assessments of clients' needs, strengths, and challenges.
  • Develop, implement, and monitor individualized care plans in collaboration with clients, families, and other service providers.
  • Facilitate access to necessary services, including healthcare, housing, education, employment, and social support.

Requirements:

  • Master's degree in a related field or 6 years of experience in direct case management.
  • Excellent interpersonal and communication skills.
  • Basic knowledge and understanding of homelessness issues, social services, and community resources in San Francisco.
  • Familiarity with trauma-informed care, cultural competency, and client-centered approaches.
