Key Responsibilities- Deliver patient-centered care, working in collaboration with physicians and other clinicians to ensure seamless care transitions.
- Conduct thorough assessments, develop treatment plans, and implement evidence-based practices to optimize patient outcomes.
- Provide education and counseling to patients and families, promoting health literacy and self-management skills.
- Participate in quality improvement initiatives, contributing to the development of policies and procedures that enhance patient care and safety.
- Maintain accurate and detailed records, ensuring compliance with regulatory requirements and hospital standards.
- Current certification as an APRN in the State of Ohio or eligibility for licensure as a PA-C.
- Graduate of a Nurse Practitioner or PA-C program accredited by a national certifying organization.
- Minimum three years of experience as an Advanced Practice Provider.
