About the Role
We are seeking a highly motivated and compassionate Homeless Services Specialist to join our team in the Rapid Rehousing Program. As a key member of our team, you will play a vital role in helping individuals and families achieve permanent housing stability.
Job Summary
The Homeless Services Specialist will provide assessment, monitoring, planning, linkage, and advocacy for individuals and families enrolled in rapid rehousing services. You will work closely with clients to develop personalized housing stability plans, connect them with community resources, and provide ongoing support to ensure successful transition into permanent housing.
Responsibilities
- Screen households to determine eligibility and document their information
- Conduct assessments to identify client strengths, needs, abilities, and preferences
- Develop Housing Stability Plans addressing crisis intervention, potential barriers, housing identification, move-in, self-sufficiency, and budgeting
- Monitor progress daily, maintain accurate documentation, and provide referrals to community resources as needed
- Work directly with the Coordinated Entry System, conduct home visits, and collect necessary documentation for client files
- Process requests for rental assistance, application fees, and other forms of assistance
- Utilize HMIS, coordinate with the Coordinated Entry System, and attend regular meetings of RRH providers
Requirements
To be successful in this role, you must possess:
- A minimum of a bachelor's degree in social work or a related field
- At least 2 years of experience serving homeless or at-risk families and individuals in crisis
- Familiarity with wrap-around services and providers within the CoC
- Ability to comply with complex governmental regulations, policies, and procedures
- Proficiency with data management and information systems, including basic knowledge of Excel, PowerPoint, and Outlook
- Knowledge of RRH Standards (HPRP, Emergency Shelter Grant (ESG), CoC, Grant Per Diem (GPD), etc.)
Benefits
We offer a competitive salary range of $55,000 - $65,000 per year, depending on experience. Our benefits package includes:
