The Opportunity

We're seeking a compassionate and driven Medical Assistant - Home Health Specialist to support our patients with care outside of traditional clinic settings. As a Field Care Coordinator, you'll facilitate telehealth visits with providers in patients' homes, building trusted relationships and assessing home safety.

You'll also assist in teaching patients and family members to use technology to coordinate care with their care teams, as needed.

Key Responsibilities:

  • Facilitate telehealth visits in patients' homes, ensuring complete views of living situations and enabling provider exams via video
  • Conduct standard patient assessments and documentation to facilitate complete visits by treating providers
  • Clinical duties may include providing best-in-class telehealth setup services, performing extended scope duties such as clinician-guided physical exams and health status screenings
  • Coordinate care with patients, families, and other outside care teams (specialists, home health, etc.) as needed

Requirements:

  • At least 1 year of experience as a Medical Assistant required
  • 1+ years of experience working in home-based healthcare services or senior living care settings preferred
  • Graduated from a Medical Assistant Program accredited by CAAHEP or ABHES, or completed relevant Medical Assistant training by any branch of the armed forces or a certified medical assistant certification

We offer a competitive salary range of $47,200 - $52,800 per year, based on experience. This role is full-time, Monday through Friday, with flexibility to travel throughout the Phoenix area. Mileage will be reimbursed at the IRS/Federally established rate.
