Key Responsibilities- Conduct thorough assessments of patients' symptom presentations, professional input, and available data to implement effective treatment plans.
- Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care coordination.
- Provide individual, family, and group therapy sessions, as well as activity therapy and educational groups, to promote patient well-being and empowerment.
- Collaborate with multidisciplinary teams to identify and address patient needs, ensuring timely and efficient discharge planning.
- Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of treatment plans and progress.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Maintain accurate and detailed records of patient interactions, progress, and treatment plans.
- Master of Social Work degree required.
- Current licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) required.
- Minimum one year of experience in a similar position and/or industry preferred.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
