Social Worker

The Social Worker is a key member of the healthcare team, responsible for providing high-quality patient care and support. This role requires a strong understanding of patient assessment, guidance, and discharge planning.
Key Responsibilities- Conduct thorough patient assessments to identify needs and develop effective treatment plans.
- Collaborate with multidisciplinary teams to ensure seamless patient care and support.
- Provide education and resources to patients and their families to promote understanding and adjustment to diagnoses and treatments.
- Identify and connect patients with community resources and support services.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Master of Social Work degree required.
- Current licensure as a Social Worker or ability to work under the guidance of a Licensed Social Worker.
- Minimum one year internship or experience in a similar position and/or industry preferred.
