The MDS Coordinator Role
As an MDS Coordinator, you will be at the heart of resident care, ensuring accurate assessments and individualized care plans. You'll be responsible for coordinating the Minimum Data Set (MDS) process, crucial for care planning and compliance. Your role directly impacts the quality of care and financial reimbursement, making it essential to both residents' well-being and our success.
Responsibilities:
- Coordinate the MDS process to ensure accurate assessments and individualized care plans.
- Manage multiple assessments and meet tight deadlines consistently.
- Analyze MDS data to optimize care planning and improve quality measures.
- Develop, review, and adjust individualized care plans based on resident assessments.
- Lead and participate in resident care conferences, fostering clear communication between staff, residents, and families.
Qualifications:
- Active RN or LPN license in New York State, with a preference for candidates with AANAC certification (Resident Assessment Coordinator Certified).
- Minimum 2-3 years of experience as an MDS Coordinator in a skilled nursing facility, with comprehensive knowledge of MDS 3.0, care planning, and reimbursement processes (PPS, PDPM).
- Strong understanding of federal and state regulations governing MDS and skilled nursing facility operations.
Compensation:
- A competitive salary range of $115,000 - $125,000, reflecting your expertise and qualifications.
