**Clinical Pharmacist Opportunity at Veterans Affairs, Veterans Health Administration**

We are seeking a highly skilled Clinical Pharmacist to join our team at the Veterans Affairs, Veterans Health Administration. As a Clinical Pharmacist, you will play a critical role in providing patient-centered care and medication management services to our patients.

Key Responsibilities
  • Participate in multidisciplinary team rounds to provide consultative services and foster evidence-based medication use.
  • Manage medication protocols and monitor patient response to therapy, providing recommendations for treatment alternatives or additional interventions.
  • Interview and evaluate patients to assess appropriateness of pharmacologic therapy and develop pharmaceutical care plans.
  • Interpret and evaluate inpatient and outpatient medication orders for appropriateness, checking for patient allergies, drug interactions, and duplication.
  • Identify and resolve prescribing and dispensing issues, including pharmacotherapeutic and formulary management.
  • Document interventions and provide safe and appropriate dispensing of medications.
  • Provide orientation and training to new employees, including technicians, volunteers, and pharmacists.
  • Serve as a preceptor for pharmacy clerkship students and pharmacy residents.
  • Maintain professional competency on drug therapies and innovations in pharmacy practice.
  • Provide medication counseling to patients and caregivers, evaluating their ability to understand medication instructions.
  • Assist in the development, coordination, and provision of drug therapy in-service programs, staff development programs, patient education programs, and other system-wide drug information programs.
Requirements
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
  • Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States, or the District of Columbia.
  • Ability to communicate orally and in writing to both patients and health care staff.
  • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
  • Ability to monitor and assess the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Working Conditions

This is a full-time position with a regular schedule, with rotating evening, overnight, weekend, and holiday coverage as determined by the supervisor.

The selected candidate will be required to serve a probationary period and pass a pre-employment physical examination.

Participation in the seasonal influenza vaccination program and the Coronavirus Disease 2019 (COVID-19) vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
