About the Role

We are seeking a highly skilled Licensed Master Social Worker to join our team on a full-time basis. As a key member of our multi-disciplinary team, you will provide essential social work intervention to patients and clients, collaborating with healthcare professionals to develop and implement effective care plans.

Key Responsibilities
  • Provide counseling and crisis intervention to patients and clients with social or emotional problems
  • Assess patient psychosocial needs, plan, evaluate, and modify interventions to meet established goals
  • Collaborate with all disciplines to plan and evaluate team goals for each patient/client
  • Facilitate patient/client and family referrals to community agencies when appropriate
  • Provide financial counseling to achieve treatment goals established by the multi-disciplinary team
  • Respond to crisis situations, including domestic violence and child abuse
  • Provide witness services and evidence in court cases
  • Visit schools to assess academic concerns
  • Work with government agencies to help clients apply for Medicare and Medicaid when appropriate
  • Arrange home visits to evaluate patient/client health, home environment, and family interaction/support
  • Assist patient/family/caregivers in planning for present and future psychosocial needs
  • Observe and report patient progress and/or changes in patient status to physician and appropriate multi-disciplinary team members
Requirements
  • Master's degree in Social Work from a school of social work accredited by the Council on Social Work Education
  • Active license as an LMSW in the state of practice
  • Minimum of one year of social work experience preferred
  • Complies with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program when applicable
  • Current CPR if applicable
  • TB questionnaire, PPD, or chest x-ray if applicable
  • Current Health certificate (per contract or state regulation)
  • Must meet all federal, state, and local requirements
  • Successful completion of new hire training as applicable to job site
  • Understand patient confidentiality and HIPAA requirements
  • Ability to effectively elicit/provide information to and from appropriate individuals via strong communication skills; proficiency in the English language is required
  • Computer proficiency required
  • Must be at least 18 years of age
