We are seeking a highly skilled Licensed Master Social Worker to join our team on a full-time basis. As a key member of our multi-disciplinary team, you will provide essential social work intervention to patients and clients, collaborating with healthcare professionals to develop and implement effective care plans.
Key Responsibilities- Provide counseling and crisis intervention to patients and clients with social or emotional problems
- Assess patient psychosocial needs, plan, evaluate, and modify interventions to meet established goals
- Collaborate with all disciplines to plan and evaluate team goals for each patient/client
- Facilitate patient/client and family referrals to community agencies when appropriate
- Provide financial counseling to achieve treatment goals established by the multi-disciplinary team
- Respond to crisis situations, including domestic violence and child abuse
- Provide witness services and evidence in court cases
- Visit schools to assess academic concerns
- Work with government agencies to help clients apply for Medicare and Medicaid when appropriate
- Arrange home visits to evaluate patient/client health, home environment, and family interaction/support
- Assist patient/family/caregivers in planning for present and future psychosocial needs
- Observe and report patient progress and/or changes in patient status to physician and appropriate multi-disciplinary team members
- Master's degree in Social Work from a school of social work accredited by the Council on Social Work Education
- Active license as an LMSW in the state of practice
- Minimum of one year of social work experience preferred
- Complies with all relevant professional standards of practice
- Participation and completion of Amergis' Competency program when applicable
- Current CPR if applicable
- TB questionnaire, PPD, or chest x-ray if applicable
- Current Health certificate (per contract or state regulation)
- Must meet all federal, state, and local requirements
- Successful completion of new hire training as applicable to job site
- Understand patient confidentiality and HIPAA requirements
- Ability to effectively elicit/provide information to and from appropriate individuals via strong communication skills; proficiency in the English language is required
- Computer proficiency required
- Must be at least 18 years of age
