Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at Great Lakes Integrated Network, Inc. as an Oncology Social Worker. This role will provide critical support to patients, families, and caregivers navigating the complexities of cancer care.

Key Responsibilities
  • Provide psychosocial screening and assessment for patients, families, and caregivers across the continuum of care.
  • Develop and implement individualized plans to address the unique needs of each patient, including counseling, education, and resource coordination.
  • Collaborate with the multidisciplinary team to ensure seamless care coordination and address barriers to care.
  • Participate in the development and implementation of programs to improve patient outcomes and quality of life.
  • Assess and remove barriers to care, coordinate with appropriate resources, and facilitate referrals to community agencies and services.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Masters of Social Work (MSW) degree required.
  • Valid and current LCSW or LMSW licensure.
  • Oncology Social Worker Certification (OSW-C) within 3 years of hire date.
  • Valid driver's license, with frequent travel to multiple sites.
Preferred Qualifications
  • Experience working in oncology social work or a related field.
  • Knowledge of community resources and services.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
