Job Summary

Care Coordinators at MedStar Health play a vital role in ensuring the continuity of patient care through outpatient healthcare settings. They collaborate with the healthcare team, community agencies, and primary care providers to promote effective communication among healthcare team members, including the patient, family, and significant others.

Key Responsibilities

  • Contribute to the achievement of established grant goals and objectives and adhere to department policies, procedures, quality standards, and safety standards.
  • Administer direct patient care and healthcare provider support.
  • In collaboration with healthcare providers, clinical and administrative staff, create clinical pathways and protocols for community care management.
  • Establish strong working relationships with community health and human service providers, including establishing a database of services and establishing referral networks.
  • Function as a clinical resource to healthcare practitioners, clinical staff, outside patient providers, and patient/family, and initiate referrals to appropriate support services.
  • Demonstrate knowledge of and participate in the development and promotion of clinical practices following MedStar St. Mary's Hospital Standards of Care.
  • Actively participate in Quality Improvement Processes as a basis for assisting with the implementation of changes in practice, when needed. Collect and analyze data as needed.
  • With hospital Case Management services, establish transition plans for patients who are being discharged from the hospital.
  • Participate, and demonstrate competency in, Electronic Medical Record (EMR) initiatives.
  • Establish and implement care plans for health promotion, illness prevention, and for disease self-management for all patients referred by the HEZ or other providers.
  • Assess protocols for all required case management indicators within required timeframes.
  • Document complete and clinically relevant assessments, reassessments, plans, interventions, and evaluations in the EMR.
  • Perform registered nurse functions for providers as needed.

Requirements

  • Bachelor's degree in Nursing required or completion within a year of hire date.
  • 3-4 years of clinical experience in a related area.
  • 1-2 years of experience in case management or a similar role.
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure. Current Maryland RN license or license eligible.
