Care Coordination Role

Care Coordinators collaborate with healthcare teams, community agencies, and primary care providers to ensure continuity of patient care through outpatient healthcare settings.

They promote effective communication among healthcare team members, including patients, families, and significant others.

Care Coordinators facilitate instruction to patients and families based on identified learning needs.

They also call and visit patients at home for follow-up as needed.

Key Responsibilities

  • Contribute to the achievement of established grant goals and objectives.
  • Adhere to department policies, procedures, quality standards, and safety standards.
  • Comply with governmental and accreditation regulations.
  • Administer direct patient care and healthcare provider support.
  • Create clinical pathways and protocols for community care management.
  • Establish strong working relationships with community health and human service providers.
  • Function as a clinical resource to healthcare practitioners, clinical staff, outside patient providers, and patient/family.
  • Initiate referrals to appropriate support services.
  • Demonstrate knowledge of and participate in the development and promotion of clinical practices following MedStar St. Mary's Hospital Standards of Care.
  • Participate in Quality Improvement Processes.
  • Collect and analyze data as needed.
  • Establish transition plans for patients being discharged from the hospital.
  • Participate in Electronic Medical Record (EMR) initiatives.
  • Establish and implement care plans for health promotion, illness prevention, and disease self-management.
  • Assess protocols for all required case management indicators within required timeframes.
  • Document complete and clinical relevant assessments, reassessments, plans, interventions, and evaluations in the EMR.
  • Perform registered nurse functions for providers as needed.

Requirements

  • Bachelor's degree in Nursing required or completion within a year of hire date.
  • 3-4 years of clinical experience in a related area.
  • 1-2 years of experience in case management or a similar role.
  • 3-4 years of experience in case management or a similar role preferred.
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure.
