Advanced Practice Provider

The Advanced Practice Provider at Neighborhood Healthcare plays a vital role in delivering exceptional patient-centered care and services within a dynamic team environment. This position involves providing direct healthcare services to patients under the direction and responsibility of Physicians in three main categories: direct patient care, team support, and clinical administration.
Responsibilities- Conduct comprehensive physical examinations on patients, including review of patient medical data, such as health history.
- Order, administer, and interpret test results for diagnostic tests, such as x-ray, electrocardiogram, and laboratory tests.
- Perform therapeutic procedures such as injections, immunizations, suturing, and wound care, and managing infection.
- Instruct and counsel patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, and health maintenance.
- Impact patient experiences by demonstrating courteous and helpful behavior and a commitment to accuracy.
- Assess and provide age-appropriate care to patients.
- Share accountability for overall patient health outcomes, working in coordination with care teams.
- Operate to instill confidence in our care and in our facilities to patients, fellow employees, and other stakeholders.
- Respond promptly and with caring actions to patients and employees.
- Maintain professional working relationships with all levels of staff, clients, and the public.
- Cooperate in accomplishing communicated department goals and objectives.
- Develop and implement patient management plans, records progress notes, and assists in provision of continuity of care.
- Perform in accordance with NHC practice protocols and under the guidance and supervision of the supervising MD(s), according to the terms of the Supervisory Agreement.
- Keep informed of organizational activities and promote mission and goals.
- Function at highest level according to credentials and competency.
- Master's degree from an accredited physician assistant or nurse practitioner program required.
- Valid CA Physician Assistant or Nurse Practitioner certification required.
- Valid CA DEA license required.
- Valid BLS certification from approved American Health Association provider required upon hire.
- Excellent verbal and written communication skills, including superior composition, typing, and proofreading skills.
- Ability to interpret a variety of instructions in written, oral, diagram, or schedule form.
- Knowledgeable about and experience with providing direct care to patients.
- Ability to successfully manage multiple tasks simultaneously.
- Excellent planning and organizational ability.
- Ability to work as part of a team as well as independently.
- Ability to work with highly confidential information in a professional and ethical manner.
- Ability to lift/carry 20 lbs/weight.
- Ability to stand for long periods of time.
- Follow all safety procedures as outlined in Neighborhood Healthcare's Illness and Injury Prevention Plan (IIPP) and report any injuries and/or unsafe conditions immediately.
- Maintain current knowledge of policies and procedures as they relate to safe work practices.
- Follow all safety procedures and report unsafe conditions.
- Use appropriate body mechanics to ensure an injury-free environment.
- Be familiar with location of nearest fire extinguisher and emergency exits.
- Follow all infection control procedures including blood-borne pathogen protocols.
- Maintain privacy of all patient, employee, and volunteer information and access such information only on a need-to-know basis for business purposes.
- Comply with all regulations regarding corporate integrity and security obligations.
- Report all behavior and/or activity that are unethical, fraudulent, or unlawful.
Pay range: $147k to $199k per year, depending on experience. (DOE) Neighborhood Healthcare
