Nurse Case Manager

We are seeking a skilled Registered Nurse to join our Case Management team at Conemaugh Memorial Medical Center. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest quality patient care and outcomes.
Key Responsibilities- Develop and implement individualized plans of care for patients
- Coordinate with the interdisciplinary team to maximize patient opportunities for success
- Ensure payer requirements are met and develop strategies to optimize patient outcomes

To be considered for this role, you must have:
- A current state RN license
- An associate's or bachelor's degree from an accredited nursing school
- At least 3 years of experience in acute care as a Professional Nurse or RN experience with significant acute care hospital Case Management experience
- Previous case management experience preferred
- Case management certification preferred
