Job Title: Per Diem Nurse

We are seeking a skilled Per Diem Nurse to join our team at Alternate Solutions Health Network LLC. As a Per Diem Nurse, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Job Summary:

The Per Diem Nurse will be responsible for providing direct patient care, assessing patient needs, and developing individualized care plans. You will work autonomously and be responsible for all practices and duties within the scope of practice outlined by the state.

Key Responsibilities:
  • Evaluate the treatment and medical condition of the patient according to the plan of care.
  • Individualize the plan of care and make adjustments as needs change.
  • Update all doctor orders in patient chart and administer medication as prescribed by the physician.
  • Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
  • Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care.
Requirements:
  • Registered Nurse with current license in the state of employment.
  • Minimum one-year experience as an RN in an acute care setting is preferred.
  • Hospice experience is desired.
  • Valid driver's license and auto insurance in your name as a driver.
